Check the allowlist before rejecting a missing action binding.

An action permit is issued for a registered action tool, but the invocation can request a different, unregistered tool. is_action remains true because it is derived from the persisted permit fields. The missing tool has no action_binding, so the router raises action_tool_binding_required before the allowlist branch can return permit_tool_not_allowed. Repeated requests also bypass governed idempotency.

Move the excluded-tool check before that binding rejection. Keep the existing binding behavior for a tool named by the action permit.

Review comment at @gateway/app/routers/mcp.py around lines 1478 - 1484:
Move the excluded-tool allowlist check in the governed invocation flow ahead of
the missing action_binding rejection, so a tool not named by the permit returns
permit_tool_not_allowed and follows governed idempotency handling. Preserve the
existing action-binding requirement for tools named by the permit; use
_deny_unregistered_tool_outside_permit for the excluded-tool check.
