fix(gateway): approval gate follow-ups from #216 review - #221
Follow-ups from Codex's review 5641350737 on #216. No migration; alembic heads is unchanged by this PR (main now has 046 on top of 045).
Duplicate-call key: a first call reuses an active (pending / approved-unconsumed, unexpired) record for the same idempotency_key instead of minting a new nonce. consume/aconsume refuse via a NOT EXISTS subquery in the same atomic UPDATE when a sibling with the same key is already consumed with a live deadline (mirrored in the memory store). Residual: two concurrent first calls can still both mint, and two racing consumes on different nonces can both pass NOT EXISTS; closing that fully needs a unique constraint (migration) — left as follow-up.
CLI admin key: amw_approvals.py refuses (non-zero, no request, key never printed) unless the URL is https or http to loopback (localhost, 127/8, ::1). Rejects userinfo tricks, look-alike hosts, other schemes, empty host. 7 allowed + 12 refused parametrized cases.
Clock: store stamps decided_at/consumed_at from the gate's now, no utc_now() left in the module. Gate expiry is now now >= deadline, agreeing with SQL deadline > now; exact-deadline tests for gate and store.
Consumed receipts: not wired to ReceiptService — it needs permit/wallet/ledger identity and an async DB session the sync gate path can't safely use. All gate events, including consumed, keep going through one default sink (documented); a test proves consumed and denied follow the same path. Durable routing belongs with the hook/mount PR.
Sync/async dedupe skipped: delegating sync to async via _run_sync would regress in-loop sync callers on the memory store.
Local: 102 passed in the three approval test files; ruff and mypy clean.
Note
High Risk
Changes tool-approval spend semantics, idempotency, and atomic consume in security-sensitive gateway code; CLI changes reduce credential exfil risk but misconfiguration could block legitimate admin use.
Overview
Follow-up hardening for the AMW approval gate and the local amw_approvals CLI.
Approval gate / idempotency: Repeat first calls for the same idempotency key now reuse an active pending or approved-unconsumed record (reissue pause) instead of minting another nonce. While a spend is still within its deadline, new first calls reject with replayed without minting. Consume is keyed to the idempotency key: DB aconsume adds a NOT EXISTS sibling check in the atomic UPDATE and can terminalize a blocked nonce to denied; the in-memory store mirrors that. Store lookups find_active_by_key / find_consumed_by_key back both paths.
Clock: Expiry is now >= deadline (aligned with SQL deadline > now). decided_at / consumed_at use the gate’s injected now, not wall utc_now().
CLI:amw_approvals.py validates the gateway URL before reading the bootstrap API key—only HTTPS or HTTP to loopback—and uses trust_env=False so proxies cannot capture the key.
Receipts: Default sink behavior is documented; consumed stays on the same logging path as other gate events (not ReceiptService).

Follow-ups (deferred to the unique-constraint PR)
Two concurrent first calls can both mint, and racing consumes on different nonces can both pass NOT EXISTS under READ COMMITTED.
In the DB store, a consume refused for a spent sibling is moved to denied by a second UPDATE. If the process dies between the two UPDATEs, the row stays approved (still refused while the spent sibling is live). A partial unique constraint on the live key closes both.
- CLI refuses to send the admin key unless the URL is https or http loopback
- single clock source; expiry is now >= deadline, matching the SQL guard
- reuse live approval per idempotency key; consume refuses if a sibling with the same key was already consumed
- document why gate receipts (incl. consumed) share one default sink

Walkthrough
Approval storage and gate paths now support timestamp-aware saves, reuse live approvals for repeated pauses, and enforce deadline and single-consumption checks. The approvals CLI validates destinations before reading or sending the API key and disables environment-proxy use. Tests cover approval behavior, receipt logging, and permitted and refused destinations.
Concurrent requests can still produce two approvals and allow the same action to execute twice. Resolve that race before merging, or explicitly accept the residual risk.
Security architecture risk: Low · up to 0e6f4
The changes tighten replay handling and credential transport without an observed new authorization bypass. Concurrent duplicate consumption remains incompletely prevented, but that exposure predates this PR. Production tool-dispatch integration and crash recovery are not established by the reviewed changes.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The sensitive outcomes are disclosure of the CLI’s configured bootstrap-admin key and authorization of irreversible tool calls. The reviewed changes do not establish tenant-wide, cross-service, infrastructure, or secret-store authority beyond those gateway responsibilities.
Security Findings and Attack Paths
inferred — Database transactions consuming different approved nonces for the same key can still both observe no consumed sibling; the added NOT EXISTS predicate alone does not establish key-wide serialization. This is residual exposure, not an observed PR regression: the base permitted both nonces to consume without any sibling check. Effective concurrency behavior under the deployed isolation level remains unverified.
Trust Boundaries and Controls
observed — The CLI accepts destination input from an argument or environment setting, validates it before reading the key, and repeats validation at client construction. It rejects malformed destinations, userinfo, and non-loopback HTTP; HTTPS remains operator-selected rather than gateway-host allowlisted. Environment-proxy trust is disabled. These controls narrow the previous unrestricted destination behavior rather than introduce a new credential destination.
Resilience and Maintainability Implications
inferred — Consumption commits before EXECUTE is returned. Interruption after that commit can leave an approval spent without a completed tool action, but this ordering predates the PR and fails closed rather than granting another execution. Coordination with the actual irreversible side effect and recovery cannot be established because the documented integration hook is outside this change.
Hardening Proposals
proposed — Before claiming key-wide single execution, introduce database-enforced key-scoped serialization or an equivalent reservation design, and verify simultaneous first calls and distinct-nonce consumes against the supported database isolation levels.
proposed — At the production integration boundary, define recovery for consumption committed before tool completion and explicitly choose durable event routing. Preserve the rule that only EXECUTE authorizes dispatch.

CLI host check (_refusal_reason): it runs in main() before the key is read and again in _client(). Host comes from urlparse().hostname, which is lowercased and strips userinfo. https is always allowed; http only for exact localhost, 127.0.0.0/8 and ::1. Bypass attempts all refuse: localhost. (trailing dot), 127.0.0.1.evil, localhost.attacker, 0.0.0.0, ::ffff:127.0.0.1, IDN and punycode hosts, ftp/file, and missing host. Uppercase scheme and host are handled. Nothing is unsafe here.
One clock: utc_now() is gone from the gate, consumed_at now stamps the caller's now, and _is_expired uses >=, which matches SQL deadline > now. Exact-deadline tests cover both the in-memory and DB stores.
Reuse: find_active_by_key and afind_active_by_key reuse a pending or approved-unconsumed live record. Since the key covers caller, tool, args and session, an args change can't hijack a reused nonce. The sibling-consumed guard is inside the consume UPDATE, so it's atomic.
Blocking:
Open Bugbot thread (approval_gate.py:314) is real. After a consume, find_active_by_key skips the spent row and mints a fresh nonce. That nonce can be human-approved but is refused as replayed until the spent record's deadline passes, then it becomes spendable. So you get a dead approval that later comes back to life. Either return the replay or terminal result for a key with a live consumed sibling instead of minting, or make the refusal permanent. Add a test for it.
Open Codex thread (test_amw_approvals_cli.py:138): _random_port, _random_*_host and uuid4().hex.invalid generate synthetic hosts and ports at runtime, which the repo's AGENTS.md data rule forbids. Use fixed, documented values (for example RFC 6761 .invalid names and RFC 5737 or 1918 literals) in place of runtime-random data.
Not blocking (deferred, as stated): find-then-save is still a TOCTOU window for two simultaneous first calls until the unique-constraint migration lands. Consumed receipts are log-only.
State: 2 unresolved threads, CodeRabbit check still pending, and the branch is BEHIND main.

Delta review of #221: 63005dd to dafaedd (b93b631 fix, plus a merge of main 47a2a57). git show --remerge-diff dafaedde is empty.
Verified:
Spent key refused at creation. find_consumed_by_key / afind_consumed_by_key exist in both the memory and DB stores, and both evaluate and aevaluate REJECT with replayed before anything is saved, so no record is created. A consume blocked by a spent sibling moves the record to denied: in memory under the same lock, in the DB with a second conditional UPDATE (_sibling_live() must still match). Until that UPDATE commits, the row is still approved and unconsumed while a spent sibling is live, so the NOT EXISTS guard keeps refusing it. The claim is true. There is one leftover window: if the process dies between the two statements, the row stays approved, and once the sibling's deadline passes it could spend. That's narrow and goes away with the deferred unique-constraint work.
Test data: the CLI tests now use fixed RFC 2606/5737/3849 values (example.com, example.invalid, 192.0.2.1, 198.51.100.7, 2001:db8::1) with fixed ports. The only randomness left is main's existing PLACEHOLDER_KEY. This resolves my earlier blocker.
Open CodeRabbit threads (non-blocking for this PASS, but they must be resolved before the merge):
approval_gate.py (Minor, reuse before the spent check): this is real but low impact. find_active_by_key runs before find_consumed_by_key, so a key with both a live spent record and a live approved one gets a PAUSE whose retry is then denied. That's a wasted pause, not a double spend. Since the second mint is gone, it only happens through the deferred concurrent-first-call race. Swapping the two lookups is a one-line fix.
amw_approvals.py:70 (proxy): this is real. httpx.Client defaults to trust_env=True, so HTTP_PROXY/ALL_PROXY can carry a loopback-http request, including the plaintext X-API-Key, to a non-loopback proxy. Recommend trust_env=False. It's a quick win and worth doing in this PR.
amw_approvals.py (userinfo): this is not a host bypass, because the key still goes to the real host. But the PR's "rejects userinfo" wording overstates it: https://guest@example.com is allowed. Reject parsed.username/parsed.password, or reword the claim.
approval_gate.py:1161 (atomic per key): this is the known deferred unique-constraint and race follow-up, and it's correctly scoped out.
test_amw_approvals_cli.py (malformed [ IPv6 case for the ValueError branch): trivial test gap.
CI: every check is green except CodeRabbit, which is still pending. 5 threads are unresolved, and the branch is BEHIND main.
- CLI httpx client uses trust_env=False so proxy env vars never see the admin key
- CLI refuses URLs carrying userinfo; malformed IPv6 refused cleanly
- first-call path checks for a live spent record before reusing an active one

Delta review of #221: dafaedd to 2cb388b (fd261f8 fix, plus a merge of main 17f4156 bringing in #195). git show --remerge-diff 2cb388ba is empty. alembic has a single head, 046_api_key_usage_events (down_revision 045), and nothing chains off 046.
_client() uses trust_env=False, and test_client_disables_trust_env_and_ignores_proxies covers it.
The spent check (find_consumed_by_key / afind_consumed_by_key) now runs before reuse in both evaluate and aevaluate. There are 3 tests for it: memory sync, memory async and DB.
Userinfo (parsed.username / parsed.password) now refuses before both allowed returns.
There's a malformed-IPv6 case, http-malformed-ipv6-bracket.
Non-blocking:
The PR body lists the concurrent first-call race and the racing-consume race, but not the crash window between the DB consume UPDATE and the deny UPDATE. Please add one line for it.
New CodeRabbit thread at amw_approvals.py:74 (Minor): this is real but not a security issue. urlparse strips control characters (for example local\thost), and a non-numeric port behaves the same way. httpx then raises InvalidURL, which isn't an HTTPError, so the CLI shows a traceback. The key travels in headers, not the URL, and no request is sent, so nothing leaks. The fix is to validate with httpx.URL(...) in _refusal_reason or catch httpx.InvalidURL.
CI is fully green (CodeRabbit is SUCCESS). The branch is BEHIND main, and 1 thread is unresolved (the one above), so the merge waits for it to be resolved.
Refuse control characters and whitespace, validate with httpx.URL, require a numeric port, and turn httpx.InvalidURL into a clean error that never prints the key.

The CodeRabbit :74 issue is fixed. Control characters and whitespace are refused before parsing. httpx.URL (the same parser the request uses) replaces urlparse. Raw-authority checks refuse @ userinfo and non-numeric ports. _client turns InvalidURL into a clean ValueError, and cmd_list and _decide catch it without printing the key.
The :92 issue is fixed. A malformed IDNA host (UnicodeError raised when .host is read) refuses as unparseable.
Codex read-only probes found no way to send the key to a non-https, non-loopback host. That covered userinfo tricks, IPv6 brackets, percent-encoding and look-alikes.
tests/test_amw_approvals_cli.py: 37 passed locally. CI is green.
Non-blocking follow-ups (none are regressions from this delta):
The leak path predates this PR. If the API key env var holds an embedded newline, h11's LocalProtocolError quotes the header value, and the except httpx.HTTPError print puts it in stderr. The fix is to reject keys containing control characters in _read_api_key, or to stop echoing exc there.
The character check misses C1 controls (U+0080 to U+009F). httpx percent-encodes them, so this is harmless today.
http://localhost: (an empty port) passes. httpx drops the empty port, so this is harmless.
