Per-key usage events and a daily per-key summary (SEL-346) in gateway/: app/core/api_key_usage.py (best-effort event write), auth hooks, api_key_usage_events model + app/db/api_key_usage_view.py daily summary view, admin summary route in app/routers/api_keys.py, schema/boot checks, docs (docs/api-key-usage.md, docs/schema-045-rollout.md).
Migration renumbering
051_api_key_usage_events -> 045_api_key_usage_events (revision 045_api_key_usage_events, down_revision 044_receipt_resolutions, the current head here). Rollout doc renamed schema-051 -> schema-045.
Not ported / adapted
This repo has no tenant RLS layer (old migration 048): app/db/tenant_rls.py, docs/tenant-rls.md, tests/test_tenant_rls.py hunks and the migration's RLS policy branch are skipped. The summary route requires bootstrap admin.
tests/test_demo_key_funnel.py, tests/test_dispatch_reconcile_migration.py, docs/railway-customer-manifest.example.json do not exist here; skipped.
Tests reuse existing tests/test_demo_keys.py fixtures instead of the absent tests.test_demo_tenant; source test data kept.
alembic heads: 045_api_key_usage_events; ruff clean. No live PostgreSQL migration run.
Note
Medium Risk
Adds post-response DB writes on every resolved DB-key request and a new Alembic migration (046) that production must apply before deploy; failures are swallowed so APIs stay up but reporting can be incomplete.
Overview
Adds metadata-only per-call tracking for database-backed API keys: after each authenticated request completes, APIKeyUsageMiddleware appends one row (route template, method, status, latency, tenant) without affecting the HTTP response if the write fails or times out.
Auth now populates key_id and tenant on the shared CredentialAcceptance holder so usage recording aligns with rate limiting. HEAD requests log the original method via amw_original_method.
Schema 046 introduces api_key_usage_events, the api_key_usage_daily view (calls, 4xx/5xx, p50/p95 latency), and boot checks for the table and view. Operators get GET /v1/admin/api-keys/usage/daily (bootstrap admin only, bounded window/row limits). API_KEY_USAGE_RETENTION_DAYS and manual prune_api_key_usage_events() are opt-in and never scheduled. Docs clarify that api_keys.use_count is for capped-key budgets, not total traffic.
Reviewed by Cursor Bugbot for commit 7b97556. Bugbot is set up for automated code reviews on this repo. Configure here.
Review follow-up: the daily usage report now bounds reads in SQL (default 30 days / 100 rows, hard max 90 days / 200 rows). Retention is code only: API_KEY_USAGE_RETENTION_DAYS is disabled by default, and the prune function is never scheduled or called automatically. Pruning production data requires C.Lee's explicit approval.
Depends on #216: migration rechained to 046_api_key_usage_events (down_revision 045_approval_gate_approvals from #216). This PR must merge after #216; CI's Alembic head/chain checks may fail until #216 lands.
Port of archived agentmiddleware/agent-middleware-api#841. Migration 051 renumbered to 045 (down_revision 044_receipt_resolutions).

It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The change adds metadata events for authenticated API-key requests with a resolved key and registered route. A daily database view aggregates call counts, status counts, latency percentiles, and timestamps. A bootstrap-admin endpoint returns these aggregates with optional key filtering. The migration adds the event table and view without backfilling prior usage. Event writes use a separate session and do not replace the application response if they fail or time out. Retention is disabled by default, and pruning requires an explicit call.
The admin report may become slow as usage events accumulate. This is a bounded concern to monitor and address as traffic grows, not an established reason to block this merge.
Security architecture risk: Low · up to 210a3
The new report is restricted to bootstrap administrators, and event records exclude credentials, request bodies, and raw URLs. Recording is best-effort and remains separate from key-use budget enforcement. Operational risk remains around indefinite metadata retention, report workload growth, and unvalidated production migration and recovery behavior.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
observed — The new dataset spans resolved database-backed keys and tenant labels. Its application report is global administrative reporting, not a tenant-scoped endpoint. This repository has no tenant RLS layer; direct database access remains a separate deployment privilege boundary whose effective grants were not supplied.
Trust Boundaries and Controls
observed — The mounted report explicitly invokes require_bootstrap_admin before querying. Normal database wallet and demo credentials resolve without bootstrap authority; authorization tests cover their rejection. The pre-existing open DEBUG fallback remains a local-development authority path, with production-like startup guardrails rejecting DEBUG mode.
Resilience and Maintainability Implications
observed — Usage events intentionally survive key revocation and deletion. Retention defaults to disabled, and setting a retention period does not schedule cleanup: the explicit pruning helper deletes at most 1,000 expired rows per invocation. This establishes manual lifecycle ownership rather than automatic erasure on credential termination.
Hardening Proposals
proposed — Before production enablement, assign an explicit retention owner and approved pruning cadence, including how repeated batches keep pace with ingestion. This would operationalize the documented manual lifecycle without treating credential revocation as historical-data erasure.
proposed — Qualify report execution at expected event volumes and consider a database execution-time budget or reporting isolation if necessary. Date and output-row limits alone should not be treated as protection for shared database capacity.

QA PASS at 8845782. Port of agent-middleware-api#841 (compared via gh; api migration 051 renumbered to 045). 045_api_key_usage_events revises 044_receipt_resolutions (main's head), no number collision. Downgrade drops the view, index and table; test_api_key_usage_migration upgrades 044->045 and downgrades back, passing locally. Local: gateway 5431 passed, 87 skipped; SDK 392 passed. Required CI green on this exact head (CI result, License result, gitleaks; Postgres proofs/concurrency pass). Merges cleanly.
Migration 045: creates a new table api_key_usage_events + index (key_id, created_at) + view api_key_usage_daily. Touches no existing/hot table, so no locks on live traffic. Downgrade drops view, index, table in the correct order. Standard Alembic (no IF NOT EXISTS), which is fine under revision tracking.
Hot path: auth only sets two fields on the existing CredentialAcceptance holder (no I/O). The event insert runs in the middleware finally, after http.response.start/body are sent, in a separate session, and every Exception is caught and logged with type name only — a usage-write failure cannot fail the request (covered by test_failed_usage_insert_does_not_fail_authenticated_request).
Privacy: only the route template (scope["route"].path), method, status, latency, key_id, tenant are stored; no raw path, query, headers, body, IP or credential. HEAD is recorded as HEAD via amw_original_method.
Tenant isolation: the summary endpoint is bootstrap-admin only (require_bootstrap_admin), parameterized key_id filter, no tenant-scoped exposure. Unauthenticated/invalid requests write nothing (tested).
Daily summary: day = DATE(created_at) over naive UTC, so boundaries are UTC midnight (documented). p50/p95 interpolation uses integer arithmetic that matches percentile_cont on both Postgres and SQLite; 4xx/5xx buckets correct.
#196 is stacked directly on this head (ancestor check OK); it adds migration 046 + reporting on top without rewriting 045.
Behind main by 14 commits; git merge-tree --write-tree origin/main <sha> is clean.
Tests (local, worktree at this SHA): test_api_key_usage.py, test_api_key_usage_migration.py, test_core_db_qa.py plus all auth/rate-limit/head-method suites — 346 passed.
Non-blocking notes
The insert is awaited inside the ASGI call after the response is sent: client latency is unaffected, but each API request holds an extra DB connection for one commit with no timeout. Under DB slowness this can drain the pool for other requests. Consider asyncio.wait_for with a short timeout, or a background queue/batch writer.
api_key_usage_daily is an unbounded view over all events (window functions, full scan). Fine at current volume; add a date-range filter on the endpoint and a retention/rollup plan before volume grows.
tenant is stored but not exposed in the view; fine for admin-only, note if a tenant-facing report is ever added.
Posted with C.Lee's approval (Oct 23, 2026, 11:14 PM PT).
gateway/app/routers/api_keys.py:79-94: the admin daily-usage endpoint has no date window or pagination, and key_id is optional. An unfiltered request selects and loads every row of api_key_usage_daily into memory.
gateway/app/db/api_key_usage_view.py:29-43: that view computes ROW_NUMBER()/COUNT(*) window aggregates over the whole append-only event table.
gateway/app/core/api_key_usage.py:34-46: one permanent row is written per authenticated request, with no retention or rollup. Together with the two items above, the report's cost grows without bound. Require a bounded date range plus pagination, or read from a rolled-up table.
CI 20/3 skip; clean; 0 threads; ff from 8845782. Local gateway 5432 passed, SDK 392. Migration 044→045 up/down covered by test_api_key_usage_migration. Observation: the declined Codex "real data" thread is resolved; the deterministic test rows are fixtures and no fixtures were removed, so no coverage lost. WARNING: #195, #196 and #61 each add a 045 revision with down_revision 044. Whichever merges second creates multiple Alembic heads and needs a renumber. #196 is based on port/api-841, not main.

Fixed: reads are bounded in SQL. gateway/app/db/api_key_usage_view.pybounded_daily_usage_sql() filters events with WHERE created_at >= :start_at AND created_at < :end_at before windowing and ends with ORDER BY day DESC, key_id LIMIT :limit. gateway/app/routers/api_keys.py caps days: int = Query(30, ge=1, le=90) and binds start_at/end_at.
Fixed: there's a new created_at index. ix_api_key_usage_events_created is in gateway/app/db/models.py and migration 045_api_key_usage_events.py, next to (key_id, created_at).
Acceptable: retention is configurable and off by default. API_KEY_USAGE_RETENTION_DAYS (default None, ge=0) is in gateway/app/core/config.py. prune_api_key_usage_events() in gateway/app/core/api_key_usage.py deletes at most one batch of 1,000 rows by created_at per explicit call, as documented in gateway/docs/api-key-usage.md:620-622.
Note, not blocking: the api_key_usage_daily view is still unbounded, but no live endpoint reads it. The admin endpoint uses the bounded query.
The DeepSeek pass timed out before writing its final verdict. I checked the items above directly in the diff.
New head (was 210a390). The only change is renumbering the migration from 045 to 046, after #216's 045.
The migration chain is correct. gateway/migrations/versions/046_api_key_usage_events.py:59-60 sets revision = "046_api_key_usage_events" and down_revision = "045_approval_gate_approvals", and the docstring says Revises: 045_approval_gate_approvals. gateway/tests/test_api_key_usage_migration.py asserts head 046_api_key_usage_events and that its down_revision is 045_approval_gate_approvals, then runs upgrade 045→046 and downgrade back. gateway/docs/schema-046-rollout.md was renamed to match.
Everything from the 210a390 review still holds. Usage reads are bounded in SQL (a created_at window, ORDER BY … LIMIT :limit, and days limited to 1-90). The ix_api_key_usage_events_created index exists. Retention is configurable and off by default, and it's only applied when someone calls it explicitly.
Brings in #216 (045_approval_gate_approvals); 046 already chains on it.

Delta review against my PASS at da7dc1e. I reviewed by hand because the Mac's load was 31, over the 28 limit, so DeepSeek wasn't run.
Merge bb727cc has parents da7dc1e and 25602d9 (main), so it's a merge with no rebase. The PR's own diff (git diff <old merge-base> da7dc1e6 vs git diff 25602d9e bb727cc1) is byte-identical except for one hunk offset in gateway/tests/conftest.py (CLEAN_DATABASE_TABLES moved from line 440 to 442 because main changed). No conflict edits.
Alembic: gateway/migrations/versions/046_api_key_usage_events.py:53-54 has revision 046_api_key_usage_events and down_revision 045_approval_gate_approvals. 045 (from main, feat(gateway): add input_required human approval gate for irreversible MCP calls #216) chains to 044. Main has no other 046, no down_revision is shared by two files, and the only head is 046.
gateway/app/core/api_key_usage.py:45 cast(Any, result).rowcount: this is a typing-only cast. session.execute(text("DELETE ...")) returns a CursorResult, and its rowcount is always an int for a DML statement on asyncpg/sqlite, never None. The function is single-batch with no loop on the return value, so a -1 driver value couldn't make it spin. The real-DB test at gateway/tests/test_api_key_usage.py:282-284 asserts 1, 1, 0. Nit, not blocking: cast(CursorResult[Any], result) would keep type checking instead of erasing it.
CI: all required checks are green: Python 3.11–3.14, Postgres proofs and concurrency, lint, CodeQL, gitleaks and CI result. The skipped jobs are matrix placeholders.
Real data: the delta adds no fixtures or synthetic data. The test files are unchanged from the head I passed.
