Expand documentation

Follow-on issue from #22, implemented in #53, which improved documentation significantly but still left some work to do. In particular, the sections on running xcetool and on testing Application Packages are currently very brief and need to be expanded considerably.

Also, the sidebar index in the generated documentation currently seems to be ordered alphabetically by source file name -- this needs to be fixed!
