fix(api): queue same-org box admissions on an advisory lock - #2036
Conversation
Hosted create counted the organization's active boxes and inserted inside one SERIALIZABLE transaction. Concurrent same-org creates read the predicate each other writes, so Postgres aborted all but one or two per round with 40001, and the five-attempt retry budget (under 100 ms) turned a burst into 503 "admission remained contended". A 50-way burst against real Postgres lost 20 or more creates per run. Admission now takes pg_advisory_xact_lock(hashtext(organizationId)) under READ COMMITTED before counting, so same-org creates wait their turn instead of aborting; each statement's fresh snapshot sees the previous holder's committed box, keeping the plan limit strict. A 5 s lock_timeout bounds the wait and maps to the existing 503 with Retry-After 5. The SERIALIZABLE retry loop is removed. Rejected: a larger retry budget (about 60 rounds for 100 creates) and Redis-held quota (a second source of truth that needs in-flight tracking to stay strict). If your organization's extra usage balance is empty, an organization admin can add extra usage credits at claude.ai/admin-settings/usage. If its monthly spend limit was reached, an admin can raise it on the same page. If neither applies, contact Anthropic support.
Once extra usage is available, someone with write access to this repository can comment @claude review on this pull request to trigger a review.

835-838: 📐 Maintainability & Code Quality | 🟡 Minor | ⚡ Quick winCorrect the bound described for a stuck lock holder.
The text says that a stuck holder pins the pool "for the full 5 s timeout".
lock_timeoutlimits only how long each waiter waits for a lock. It does not limit how long the holder's transaction runs. Nostatement_timeoutoridle_in_transaction_session_timeoutis set inwithCreationAdmission.A stuck holder therefore keeps the advisory lock until its transaction ends. Each waiter holds a pooled connection for up to 5 s, then returns 503. New creates for that organization keep taking connections the same way. As written, operators can read 5 s as the maximum length of the outage.
📝 Proposed wording
-queries on that pool waited up to 0.9 s. A stuck lock holder can pin the pool -for the full 5 s timeout. +queries on that pool waited up to 0.9 s. Each queued create holds its +connection for at most 5 s; a stuck lock holder is not bounded by that +timeout, so creates for its organization keep occupying connections and +returning `503` until the holder's transaction ends.🤖 Prompt for AI Agents

SET LOCAL lock_timeout applied to the whole admission transaction, so a warm-pool claim waiting on a Box row lock held by updateWhere() aborted after 5s and surfaced as the admission-contention 503. Set the timeout, take the lock, and restore the default in one parameterized statement, and map 55P03 to 503 only for that statement. The lock now uses a two-key form with a namespace constant. 🩺 Stability & Availability | 🟡 Minor | ⚡ Quick winSplit the timeout setup, advisory lock, and restoration into ordered statements.
withCreationAdmissionreaches this SQL for creates and warm-pool assignments withmaxCreatedBoxes. PostgreSQL does not guarantee the evaluation order ofSELECTtarget-list expressions. If restoration runs beforepg_advisory_xact_lock, admission contention can use the previous session timeout instead of 5 seconds. If the 5-secondset_configruns last, it can remain active for the later count orBoxrow update. The real-PostgreSQL tests cover the current executor behavior, but cannot establish an ordering guarantee.Suggested fix

209-210: 🚀 Performance & Scalability | 🟡 Minor | ⚡ Quick winLimit per-process admission concurrency before opening the transaction.
withCreationAdmissionopens a TypeORM transaction before it waits onpg_advisory_xact_lock. The waiting transaction keeps its pooled connection. A burst from one organization can therefore occupy the shared pool for up to the 5-second lock timeout. Other database-backed requests and/health/readychecks can wait for a connection. The liveness endpoint does not use the database.
lock_timeoutlimits each wait. It does not limit the number of connections that can wait. Add a per-process, per-organization semaphore beforedataSource.transaction, with a small limit such as 1–2, and release it on every completion and failure. Keep the advisory lock for cross-instance serialization. Also record admission-lock wait time andBoxCreationAdmissionUnavailableErrorrates.🤖 Prompt for AI Agents

TL;DR
Same-org create bursts now queue for admission instead of failing with 503.
