fix: tighten quality guardrails for issue 169

[TATP-233]

Fixes #169

Impact Scope

• tighten docs validation so issue templates are checked for script paths and Hydra semantics
• make play_interactive use shared entrypoint log-root semantics for temporary runner state
• raise the default CI non-slow coverage floor from 10 to 25 and document the ratchet policy
