Check 156: description_has_changelog_entry

Fires when a tool or parameter description contains version-history language like "Added in v2.0", "Deprecated since version 3", or "Available since v1.5".

Why it's a problem

Tool schemas are specifications, not release notes. Changelog entries:

• Are invisible to models at inference time (they don't know what version they're running)
• Waste tokens every time the schema is included in a context window
• Belong in CHANGELOG.md, not in description fields

Fix
